Considering that the title is a pun: cholera as the disease, and cholera as passion, raises the central question of the book: is love helped or hindered by extreme passion?
The Narrator is extremely seductive alot like Wrong Hands.

😉 So the comment about the smell is from Captain Diego Samaritano of the riverboat on which Fermina and Florentino ride at the end of the novel?!
And is it Florentino who says he doesn’t feel well (because he forgot his little blue pills)?!
